2024 Staff Review

The annual staff review provides an opportunity for us to…
  • Look back at the previous year. We’ll evaluate goals, celebrate successes, and discuss challenges.
  • Look forward to next year. We’ll set goals to ensure organizational alignment and create development and performance plans.
  • Monitor team and organizational culture. We’ll holistically evaluate the health of our staff as we review feedback on individual, team, and organizational issues.

Who Completes This: Full-time staff and regular part-time staff are required to complete the review process. It is recommended that other part-time staff also participate in the staff review, but not required. If you are part-time and are not sure you need to complete a staff review, check with your supervisor.

Deadlines for Submitting Staff Reviews:
  • All employees submit reviews by Friday, November 15
  • Final approval and submission by Friday, December 13

STAFF REVIEW PROCESS

BEFORE MEETING # 1

  1. Employee should ask supervisor for initial direction on goals to be set for next year to find out any desires or expectations that need to be incorporated into the plan.

  2. Employee completes Staff Review.

  • Go to the Annual Staff Review eform found on the Intranet under Quick Links / eForm / Annual Staff Review.
  • Complete only Sections I & II. (Section III is to be completed by supervisor.)
    • If needed, find last year's goals in the eForm titled “Annual Staff Review – LAST YEAR”. Make sure you select the eye icon next to your name to see the Review.
Important Notes
  • You must be logged into the Network to Submit your eForm.
  • Ideally, fill this out in 1 sitting to avoid losing progress. Always SAVE DRAFT if necessary to pause.
  • Once you SUBMIT, the form will be locked and you will be unable to edit. Your supervisor will be notified via email.
  1. Employee and Supervisor coordinates a time for Meeting # 1. (This often takes place during a normal 1:1 meeting.)
MEETING # 1
  • Employee will lead this meeting by discussing responses within the staff review. Supervisor primarily listens.
  • Employee and Supervisor discuss and refine Individual Goals for next year if needed.

BETWEEN MEETING 1 AND 2

  1. Supervisor will gather necessary information on each employee from upline.
    • Note: It’s the responsibility of each supervisor to gather this information but department heads, divisional leaders, etc. are encouraged to proactively reach out directly to all supervisors regarding anything they feel should be included in reviews.
  1. Supervisor completes Section III of staff review incorporating feedback from each employee's upline.
    • Note: Supervisors can choose to SAVE DRAFT upon completion of the form and click SUBMIT at the beginning of Meeting # 2. You must click on "Save Changes" before you make the approval. If the approval pop-up box covers up the Save Changes button, then click on "Minimize".
  1. Supervisor and Employee coordinate a time for Meeting # 2. (This often takes place during a normal 1:1 meeting.)

MEETING #2
  • Supervisor leads discussion over Section III.
    • Discussion regarding meeting of all Expectations.
    • Comments regarding Accomplishment and Performance.
    • Comments regarding Growth and Development.
  • Individual Goals for next year should be finalized during this meeting.
  • Supervisor Approves.
  • Employee Approves (acting as a Signature) OR may choose to wait if desired.

FOLLOWING MEETING # 2

  • Upline reads all staff reviews from employees in their downline.
  • Human Resources reads staff reviews from all employees.


To save/print your review:

Go to the form on the Intranet and click on "Responses". Next, click on the eye icon next to the name of the employee's review that you want to save. In the top right corner of the review you will see an Adobe Acrobat icon. Click on that icon and it will immediately save the PDF to your downloads folder on your computer.

Additional Notes:

Supervisor, if you need to send the review back to the employee for edits, please select Decline and this will send it back to the employee to make edits and then employee can re-submit it. (If you choose Reject, the employee will not be allowed to edit it.)
